Please be informed that starting June 1, 2026, the single musculoskeletal tissue request form M02/PO04 will come into effect, valid for both orthopedic surgery and dentistry (fillable PDF).

The form is available in the Restricted Area, under the Applicants - Forms section.

Simultaneously, the Fee Schedule has been revised, updating tissue reimbursement rates and product identification codes.

Please note that the updated rates will apply to all shipments made from June 1, 2026.
